1. Purpose Mindful Growth Counseling is committed to protecting the privacy and confidentiality of client information. This document explains how personal and health information is collected, used, stored, and protected. 2. Information Collected Personal information may include name, date of birth, contact information, emergency contact, and insurance information (if applicable). Protected Health Information (PHI) includes mental health history, treatment notes, diagnosis, and session records. 3. Use of Information Information is used to provide counseling services, maintain clinical records, process payments, communicate about appointments, and comply with legal and ethical requirements. Information is never sold or shared for marketing purposes. 4. Confidentiality & HIPAA This practice follows federal and Oregon state confidentiality laws, including HIPAA. Information will not be disclosed without written authorization except in situations involving risk of serious harm, suspected abuse or neglect, court order, or as otherwise required by law. 5. Storage & Protection Client records are stored using secure, password-protected and encrypted systems. Reasonable administrative, technical, and physical safeguards are used to protect client data. 6. Telehealth & Electronic Communication Telehealth services are conducted through HIPAA-compliant platforms. Email and text messaging are used primarily for scheduling purposes. Electronic communication carries some risk, and confidentiality cannot be fully guaranteed. 7. Client Rights Clients have the right to access their records (with legal exceptions), request corrections, request limits on disclosures, and file complaints without retaliation. 8. Record Retention Records are maintained in accordance with Oregon state law and professional standards and are securely destroyed after the required retention period
